NEW PATIENTS WITH PRIVATE INSURANCE OR NO INSURANCE

• $50.00 for the initial intake - Covers 1st, 2nd and 3rd day

• $15.00 per day (Buprenorphine) starting 4th day

• $8.00 per day (Methadone) starting 4th day

 

ESTABLISHED PATIENTS WITH PRIVATE INSURANCE (Cigna, UHC, ETC)

Patient will be responsible for the $15.00 a day until insurance starts paying medical expenses at 100%. At that time the patient will still be responsible for the daily medicine charges. 

 

You may choose to pay daily or in advance. We will not be able to extend credit. All copays and medicine money is due at the time of service. 

 

If your insurance does start to pay at 100% you may be eligible for a refund on the services that you have paid for. Medicine money is non-refundable. 

 

MEDICARE AND MEDICAID PATIENTS

• Medicaid patients are responsible for $8.00 a month in copays (two doctor visits and two drug screens). 

• Medicare patients are responsible for their co-pays.

 

NOTE: The daily rates of $8.00 and $15.00 include medication. No prescription plans are accepted.
